Tokyo offers small hospitals up to ¥2.7mn to fund one discharge-support nurse

Tokyo hospitals with fewer than 200 beds can claim up to ¥2.7mn for one discharge-coordination nurse or social worker, but the higher tier requires a record of home-care admissions and shared records with community providers.

Tokyo is paying part of the wages of the nurse or social worker who gets patients out of hospital and into home care. The metropolitan government's subsidy for admission-and-discharge coordination covers one such employee per hospital, with a cap of ¥1.8mn or ¥2.7mn depending on how much the hospital does beyond the basics.

Who qualifies

The scheme is open to hospitals in Tokyo whose beds total fewer than 200 under the Medical Care Act. Three groups are excluded: hospitals that held the higher-tier admission-and-discharge support fee under facility standards on 1 April 2026, hospitals with only psychiatric beds, and hospitals run by local governments, independent administrative bodies, national universities or the state.

The covered cost is the personnel cost of one nurse or social worker, assigned to discharge support, who either completes a 2026 admission-and-discharge coordination or discharge-support training course, or finished an earlier course. Staff in the second group qualify only if prior subsidy payments for them under related Tokyo schemes total no more than three years. Salaries, bonuses, allowances and statutory welfare costs count; costs already covered by other national or local subsidies do not.

Two tiers

The base amount is ¥3.6mn per hospital. A hospital that meets the first three requirements, covering stronger admission-and-discharge coordination, work on home-care transition and local medical-care links, and arrangements to take back home-care patients whose condition changes, gets half of eligible costs, up to ¥1.8mn. A hospital that also meets two further requirements gets three-quarters, up to ¥2.7mn.

The two extra requirements are the demanding ones. One is a record of accepting nine or more home-care patients. The source says a hospital must show nine or more over the most recent three months, both when it applies and when it files its April results report. The other is using a multi-professional collaboration system to share information with local medical and care workers. The requirements apply to the year from 1 April 2026 to 31 March 2027.

Deadline

The listing gives an application window of 14 October to 31 December 2026, but the description states a closing date of Friday 18 December and asks late applicants to contact the Tokyo Metropolitan Government's medical policy division. Hospitals should treat 18 December as the working deadline. Applications are entered directly into an online form, and applicants no longer need to prepare the prescribed forms.
